Specialist in-home clinical support

Our expert clinicians offer a range of nursing services in your own home to assist you to manage your health and wellbeing. We can also support you to recover safely and comfortably at home after hospitalisation.

Access to clinical services in your own home can help you feel comfortable, safe and well cared for in a familiar environment, and help you access the support you need while reducing the number and frequency of your external medical visits. We will work with you to develop a tailored visit schedule by our nurses or other highly qualified support workers to attend and provide the support you need.

Disability support schemes explained

We offer private services if you are not eligible for government or insurance funded disability services, want to top up your funded services with extra assistance, or want to self-manage your support services following payment of a permanent impairment benefit. There are no waiting periods and you can organise a quote for services directly with our team.

Immediate access to self-funded services

Programmed Care's private services can be booked immediately with no wait times or eligibility assessments to deal with. With private services you have full control of the amount, type and timing of the services you wish to purchase.

Many people choose to book Programmed Care's private services as an alternative or supplement to funded care, allowing them to bypass the formalities and get the care they need right now.

With a wide range of service options, specific regular assistance can help you achieve your goals and be well connected to the people and things that matter most to you.

The NDIS invests in support and services for people under the age of 65, living with a permanent or significant disability. The NDIS is a government funded initiative, which aims to improve long-term outcomes for participants

The NDIS provides three broad categories of supports for people living with disability:

  1. Core Supports including assistance with daily living, consumables and transport
  2. Capital Supports including assistive technologies and home modifications
  3. Capacity Building Supports including support coordination, community access and participation

 

Accessing Supports

If you meet the eligibility criteria and you would like to become a participant, you will need to contact the NDIS to make an Access Request. The Access Request form will ask you to provide your personal details, contact information and details about your disability. You can provide copies of existing information, including letters or reports, or you can ask your treating health professional to assist. In some cases the National Disability Insurance Agency (NDIA) may ask you to provide evidence of your disability, this includes information on what your disability is, how long it will last and how it impacts your life. You can work with your treating health professionals to provide your evidence of disability.

Access Decision

Once the NDIA has finalised your access request, they will then send you an ‘Access Decision’ letter to tell you the outcome of their decision. If you’re approved you will work with a NDIS Planner or a Local Area Coordinator to develop your written support plan. The planner will ask questions about how you are going in different areas of your life, what goals you would like to achieve and what kind of help and support you need. This will help them to develop a plan that provides the right support for you.

Activating and managing your plan

A Local Area or Support Coordinator will then help you activate your plan. You have a range of options for managing your plan – from self-management through to specialist management by a provider. If you choose to work with a NDIS provider, you will need to make a written Service Agreement with your provider. This agreement will ensure your provider delivers the right supports in the right way.

NDIS Registered Provider

Each state of Australia has a government funded insurance scheme that provides treatment, care and support for people who have sustained serious and permanent injuries in a motor vehicle accident, regardless of who was at fault.

The insurance scheme in each state is funded by Compulsory Third Party (CTP) insurance, which is a levy paid when registering your car. Although this is a national scheme, some states vary on the basis of other factors, including fault, liability, injury and compensation. Anyone who sustains a serious and permanent injury in an accident involving a motor vehicle may be eligible to receive necessary and reasonable lifetime treatment, care and support under the Scheme.

For each claim, the respective state based agency will assess the participant’s treatment, care and support needs as a result of the participant’s injury. The assessment must relate to the treatment, care and support needs that are necessary and reasonable in the circumstances and relate to the motor vehicle injury.

If you are approved for support at home, a support coordinator from the relevant transport accident insurance agency will work with you and your treating team to develop a plan for the support you need to achieve your goals. You can then choose one or more service providers to provide your support plan.

It is important that you choose a service provider suitable for your own individual needs. Providers are reviewed and accredited against specific quality standards in order to become approved providers of transport accident insurance support programs. Providers that have met these standards have agreed to provide a particular level of service and to abide by the transport accident insurance agency policies and dispute resolution processes.

Workers’ compensation is a form of insurance payment to employees if they are injured at work or become sick due to their work.

Workers’ compensation includes payments to employees to cover their:

  • Wages while they are not fit for work; and
  • Medical expenses and rehabilitation

If you have sustained a serious or permanent injury at work, or as a result of work, you may be entitled to make a claim for compensation under state law. Through the Workers’ Compensation Scheme in your state you may be eligible to receive a range of medical, attendant care and rehabilitation supports on either a short or longer term basis.

Under your state’s safe work insurance scheme, injuries are usually assessed for the degree of permanent impairment of an individual, which will determine what you are eligible for. The assessing doctor will ask you a series of questions about the treatment you have received to date, and your current problems. Depending on the nature of your injury, the doctor will conduct a physical examination. This is to measure the effect of each injury or condition on your ability to move or function as normal.

Following the assessment, the doctor will prepare a report based on the assessment. This report will provide a percentage score describing the level of permanent impairment of your assessed injury.

If your claim is successful, the workers’ compensation program in your state may manage and pay for treatment and care to meet needs related to your workplace injury. Services may include:

  • medical treatment (such as hospital stays and appointments)
  • rehabilitation (such as physiotherapy, occupational therapy and speech therapy)
  • aids and equipment (such as wheelchairs)
  • home and vehicle modifications (such as ramps and bathroom rails)
  • attendant care services (including personal care, domestic assistance and clinical support)
  • workplace rehabilitation (including return to work programs and modifications).

The services must be for “reasonably necessary” treatment and care to meet needs related to your workplace injury.

At Programmed Care, we have a team of experts who can assist you with support coordination and any of the following core supports: 

  • Consumables – Programmed Care can help you purchase everyday items such as incontinence aids and feeding equipment. 
  • Assistance with social and community participation – supports to access your community for social and recreation activities either individually or in a group, or to attend a centre-based program. 
  • Assistance with daily activities – nursing care, personal activities, shopping, household tasks, living arrangements and 24hr care, including overnight shifts and respite care. 
  • Transport –vehicle fees and costs (including taxi fees) to transport you to and from community services or activities. These costs could include, parking fees, road tolls and vehicle running costs (eg, petrol and car maintenance). Transport does not include your support workers time to accompany you, just the additional vehicle costs.

If you wish to cancel a service booking, 2 hours’ notice is required. 

To cancel your services with Programmed Care all together, we require at least 24 hours’ notice 

We work with you to ensure we find the right support workers to match your requirements and build a consistent care team to limit the number of new staff entering your home. However, sometimes due to circumstances out of our control, we may need to send a new staff member. We will be sure to notify you if your usual care team is unable to make your scheduled shift. 

At Programmed Care, our main goal is to ensure everyone has equal opportunities to receive the care they require. If your current agency cannot meet all of your needs, we are happy to work with them to provide support in areas they are unable to fill.  

Programmed Care work with you to source suitably qualified staff. If we do send someone you don't connect with, please let your care consultant know and we will make arrangements to replace them with someone more suitable.
