Two workers were observed operating a boom lift without being connected to their harness anchor points, and the operators were also not appropriately qualified to operate the equipment.
Key Controls
* Ensure all boom-type EWP operators are appropriately licensed, trained and authorised.
* Wear a correctly fitted harness and remain connected to the designated anchor point whenever the EWP is elevated, being elevated or travelling.
* Use a twin-tail lanyard when changing anchor points to maintain continuous attachment.
* Review and follow applicable SWMS, SOPs and site requirements before commencing work.
* Inspect the EWP and required PPE before use.
* Conduct buddy checks to verify workers are correctly connected and challenge unsafe behaviours immediately.
