Help on hand for in-home care

Being able to continue to live independently at home while feeling safe and well supported is a key goal for many older Australians.  When considering whether to stay at home or move into residential aged care, individuals and their families want to understand the type and quality of care available, how well it meets the person’s lifestyle preferences, and whether it can increase or alter in response to changing needs.

Understanding how aged home care is funded can also be challenging, with many people unaware of their potential eligibility for government funded assistance to continue living at home with the help of a Home Care Package. Home Care Packages provide in-home care and support to help older people to stay in their home and are funded by the Australian Government through Medicare. An individual’s eligibility is determined by an Aged Care Assessment Team assessor (ACAT), who will meet with the person at home to determine which level of care best fits their needs.

As an approved provider of Home Care Packages, Programmed Care is very experienced in helping older people and their families navigate the assessment, funding and care choices available, as well as being experts in the provision of a broad range of in home care services.

Programmed Care, including its wholly owned business, Clinical Care Professionals, is able to provide help around the home, assist people to get out and about and maintain their social connections, help make home cooked meals, and assist with mobility, showering and dressing.  And when expert home nursing care is needed, the Programmed Care team of fully qualified and highly experienced nurses can provide post-operative care, diabetes management, and wound and stoma care.

An individual plan is developed in consultation with the client at the initial meeting in their own home with a dedicated home care package coordinator, with all costs clearly explained up front.
